About the Project

The Transitions Clinic Network (TCN) is a national network of medical homes for individuals with chronic diseases recently released from prison.

The objective of the redesign was to provide a clean modern design that highlighted the national scope of the organization.

Key features include a custom interactive map and mobile responsive design.
